- The notice code and tax year
- The date received and the response date printed on it
- All pages of the notice, including any inserts
Tell the office when you call. A booking does not suspend collection or extend the date on a notice.

Keep documents
ready for review.
The booking form asks for limited contact information and a short description. It does not accept attachments. Do not put Social Security numbers, bank details, or complete tax records in its notes.
When records are needed, contact the office for delivery instructions. Receipt of a document and professional review are separate steps.

- Recent IRS or state correspondence, including every page.
- Tax years involved and any returns that may be missing.
- Recent filed returns and relevant payment records.
- Any existing payment agreement or prior representative.
- Questions about income, essential expenses, or business obligations.
